A comprehensive 40-minute initial evaluation to discuss your medical, psychiatric and social history, treatment goals, plan of action and any relevant info that will assist with your treatment.

Cancellation Policy

24 Hour Notice Required For All Cancellations/Rescheduling. No shows or less than 24-hour notice or reschedules will incur a $30 fee payable via cash, check or credit card only.
